Good Faith Estimate

Your right to a Good Faith Estimate
Under federal law (the No Surprises Act), if you do not have insurance or you choose not to use it, you have the right to receive a Good Faith Estimate of the expected charges for the services you are scheduling. This is a cash pay practice, so this applies to every patient.
How to get yours
You can ask for a Good Faith Estimate when you schedule, or at any time before your appointment, by emailing care@thrivewithfm.com. [Timing: confirm estimates are provided within 1 business day if booked at least 3 business days ahead, and within 3 business days if booked at least 10 business days ahead]
What it covers
- The practice's own fees for the services you are scheduling.
- Laboratory charges, medication and supplements are billed to you directly by the laboratory, the pharmacy or Fullscript, and are not part of the practice's fees. [Confirm how these are handled on the written estimate]

- Lab Review & Plan: starting at $125
- Initial Consultation: $349
- Weight Loss Program: $399
Your written estimate will show the expected charges for your specific services.
If your bill is higher than the estimate
If the bill you receive is $400 or more above your Good Faith Estimate, you have the right to dispute it. You must start the dispute process within 120 calendar days of the date on the bill. Keep a copy of your estimate and your bill.
